Major Responsibilities:
- Counsels individuals aged 16-24 using appropriate counselling techniques regarding behavior, attitude and environmental challenges as to the best alternatives and/or course of action in relation to career exploration.
- Receives referrals from partner agencies and initiates case management to resolve the reason why participants are vulnerable to employment precariousness and provides necessary assistance.
- Conducts assessments and intake interviews with youth who are facing barriers based on the deep roots of violence, are street involved, in homeless and low-income service settings and institutional settings and assesses their income, employment and housing status to determine program eligibility.
- Liaises with staff from other programs and community agencies (e.g., hospitals, social workers, doctors, courts, immigration, shelters, drop-ins, health clinics, legal programs, other City Programs, etc.) to provide program information or advocating for transfers/placements of youths.
- Liaises with non-profit service providers in the coordination and provision of services to the youth as determined in the case plan.
- Follows through on referrals as appropriate and maintains follow up.
- Advocates on behalf of youth participants for support, services and benefits with internal and external resources, and engages in active problem solving around each youth's risk factors.
- Works independently and also as part of a larger team to identify client needs and assist the client in accessing appropriate programs and services.
- Advocates on behalf of clients, including their children, and ensures client involvement in case planning and decision making processes to facilitate empowerment and personal growth.
- Monitors individuals’ progress by maintaining regular contact through counselling and discussing any change in status, needs, resources, etc.
- Develops and nurtures neighbourhood connections and recognition within youth population assigned, using community development principles.
- Provides ongoing assessment, case management, care coordination and monitoring of youth’s needs; monitors and evaluates youth's changing needs and coordinates individualized services.
- Assists youth to identify and access alternative courses of action when community-based services are not appropriate to the needs of the individual.
- Works in partnership with sector-based Community Development Officer to match youth needs with employment sectors appropriate to their skill level and interest.
- Accompanies youths to appointments as necessary or coordinates youth supports for appointments.
- Maintains a case load of all required related information including case notes, documentation, reassessments, forms, applications, etc. and develops case plans.
- Organizes and/or attends case conferences with others to resolve issues related to youth participants employment precariousness.
- Participates in the development of youth profiles and service needs by preparing various reports and statistical forms, Compiles and maintains records and related statistics.
- Submits activity logs and other reports as required.
- Ensures safety and well-being of youth accessing the program.
- Participates in program development and delivery.
- Participates in meetings to discuss program design evaluation at the City, other orders of government, community forums and recommends needed program enhancements and responses.
- Develops and coordinates workshops/events that address employability including job fairs.
- Maintains current knowledge of legislation, community services, resources, policies, programs, procedures and issues affecting vulnerable youth.
- Engages in continuous learning to ensure familiarity with changes in the unit’s policies, guidelines, philosophy and service delivery.
- Participates on internal and external committees as requested by Supervisor.
- Responds to inquiries from members of the general public, business operators and youth, etc., requesting a service response.
Key Qualifications: Your application must describe your qualifications as they relate to:
- Degree in Social Services, Social Work, or related field or equivalent combination of education and/or experience.
- Experience in counselling youth with psycho/social/physical, educational, emotional and/or environmental problems.
- Experience working from an anti-oppression framework on issues (such as diversity, access and equity, civic engagement, street gangs, violence, substance abuse, racism and harassment and other community development principles as they relate to youth and the ability to deal with these issues as they arise).
- Experience developing and implementing case management plans (i.e. youth plans to reach youth goals) specifically with youth facing employment precariousness who are homeless, street involved or facing other barriers to employment.
- Experience in providing intake, assessment, counselling, and referral and follow up support to young people.
You must also have:
- Ability to carry out cold contacts and other outreach techniques in a variety of public locations, at a variety of times, including bringing services and supports to young people who are homeless.
- Ability to utilize a variety of software packages (such as MS Word, spreadsheets, and database applications).
- Ability to work as a team member in a dynamic, complex environment and organize time and workload effectively, while remaining accountable.
- Ability to create community development youth employment activities.
- Ability to work in a customer service environment, addressing the service inquiries, requests and complaints of youth, the general public and/or staff.
- Knowledge of gaps in services for youth requiring support to enter the labour market.
- Knowledge of applicable legislation (such as the Ontario Works Act and Ontario Disability Support Program, Employment Standards Act, Ontario Human Rights Act, Municipal Freedom of Information and Protection of Privacy Act (MFIPPA).
- Ability to work flexible hours when necessary.
- Ability to travel to meet youth clinics in community.
- Ability to work as part of a collaborative and multi-disciplinary team
