Major Responsibilities:

 

  • Conducts home visits with assigned families to provide for the development of the child. Suggests practical parenting ideas, e.g., toilet training, feeding young children, as well as games and activities that help in the development of children. Helps parents recognize what children need and want
  • Schedules appointments for home visits, plans and prepares materials for teaching and demonstration of activities in the home that support the child’s development and attachment to the parent
  • Assists parents to find community resources
  • Advises parents by sharing stories about experiences of pregnancy, labour and delivery and family planning. Promotes and supports breastfeeding
  • Assists parents in the completion of growth and development screening tools
  • Completes Family Home Visitor documentation and reviews records with the Family Home Visitor Supervisor
  • Works closely with the Public Health Nurse, Dietitian and other health care professionals to support parenting with their families
  • Delivers presentations, along with health care professionals, to outside agencies and interested groups as requested. Informs and provides orientation on the program to different ethnic groups and the community
  • Orients divisional staff and students to the program, cultural practices, values and beliefs specific to ethnic groups. Attends internal meetings with peers and co-workers to provide updates and exchange information
  • Participates on committees, workgroups, etc.

 

Key Qualifications:

Your application must describe your qualifications as they relate to:

1. Post Secondary Education in child growth and development and or equivalent experience relevant to the primary duties of the position.

2. Considerable experience as a Peer educator, providing parenting educational information to prenatal mothers and mothers with children up to the age of six (6) years.

3. Experience with teenage parents, refugees, new immigrants, underhoused populations or those experiencing barriers in accessing services.

4. Fluent in one of the following languages: Spanish or Ukrainian.  

5. Experience in Microsoft Office programs and other relevant systems (e.g. Outlook, Word, ISCIS, TCHIS).

6. Possession of a valid Class “G” Ontario Driver’s License and access to a vehicle.

  • Ability to deal with confidential information, handling of confidential health records and client personal health information.
  • Excellent interpersonal skills and the proven ability to work effectively and independently as well as a member of a team.
  • Excellent organizational skills with the ability to work effectively according to procedures and timelines and proven ability to set priorities.
  • Ability to handle crisis situations and good problem solving skills.
  • Ability to travel to various locations in a timely and efficient manner.
