The City’s Seniors Services and Long-Term Care Division supports Toronto seniors and residents in long-term care homes to have the healthiest, most fulfilling lives possible through exceptional care and services. Vulnerable individuals who reside in the community are supported through adult day programs, supportive housing services, tenancy support and homemakers and nurses services. Major Responsibilities:
- Implements detailed plans and recommends policies/procedures regarding program specific requirements.
- Conducts nutrition assessments for new admissions and for residents at nutrition risk, develops nutrition plan of care with goals and strategies to meet resident's nutrition needs.
- Monitors and evaluates resident's progress towards the nutrition goals, documenting and making adjustments to the plan as required.
- Interprets and individualizes resident's diet and writes diet orders for therapeutic and texture modified diets, nutrition supplements and tube feedings and their impact on resident's quality of life.
- Provides counseling to residents/ families on various therapeutic/ texture modified diets, supplemental and tube feedings.
- Promotes activities/processes/practices in Food Nutrition services that are compliant with Divisional policies and Ministry of Long Term Care standards and report those that are not in compliance to the Home Administrators.
- Participates in multidisciplinary care conferences for residents with nutrition risk issues and communicates with the multidisciplinary team members in the development, implementation, monitoring and evaluation of the residents' nutrition plans.
- Acts as a resource for Nutrition issues for the Food and Nutrition Department and Multidisciplinary Care Team and provides in service education based on Home requirements.
- Collects and monitors risk management data and participates in QI processes.
- Participates in developing and monitoring Nutrition Care policies and procedures that are developed incompliance with Ministry of Long Term Care Standards.
- Ensures menu planning, recipe development, product evaluation and portion control standards, nutrient analysis in accordance. with resident's needs and desires, and Ministry of Long Term Care Guidelines.
Key Qualifications:
Your application must describe your qualifications as they relate to:
- Current registration with the College of Dietitians of Ontario.
- Considerable experience in Clinical Geriatric Dietetics.
- Considerable experience working in a Long-Term Care environment.
- Knowledge of the Fixing Long-Term Care Act and Divisional policies and procedures.
- Ability to plan menus in a Long-Term Care setting.
- Ability to promote and foster effective teamwork and establish excellence in a customer service-oriented environment.
- Ability to build and maintain effective relationships and partnerships with a diverse range of partners as well as work effectively within inter-professional teams.
- Excellent interpersonal skills with the ability to communicate effectively, build strong working relationships and resolve conflict with all levels of the organization.
- Familiarity with government legislation in the area of occupational health and safety.
- Ability to support the Toronto Public Service values to ensure a culture that champions equity, diversity and a respectful workplaces.
Please Note:
The role will also provide support to the Food & Nutrition department operations as required.
Note: As a condition of employment with the Seniors Services & Long-Term Care Division, selected candidates will be required to provide a satisfactory Police Reference Check (PRC) with vulnerable sector screening, tuberculosis screening and proof of immunization in accordance with the Fixing Long-Term Care Act, 2021(O.Reg 246/22).
