Job Description:
Reporting to the Associate Director, Clinical Services, the Project Manager will lead project management of the implementation of the Downtown Homelessness Addiction Recovery and Treatment (HART) Hub. In collaboration with health care and community partners, the Downtown HART Hub aims to improve the health of Toronto residents by facilitating access to health care services for people who use drugs, including improved access to treatment and recovery services, and integrated pathways to wraparound supports such as mental health care and housing.
The Project Manager will develop, execute and monitor plans for implementing the interim HART Hub approach and establishing a permanent site and model for co-located services. This position will ensure effective collaboration and facilitate action across other Directorates, Divisions and external partners and stakeholders.
Major Responsibilities:
- Implements detailed plans and recommends policies/procedures regarding program specific requirements.
- Conducts research into assigned area ensuring that the project takes into account developments within the field, corporate policies and practices, legislation and initiatives by other jurisdictions and levels of government.
- Motivates and collaborates with project team members, ensuring effective teamwork, high standards of work quality and organizational performance, continuous learning and encouraging innovation in others.
- Advances, negotiates and supports execution of the necessary funding proposals, contracts, agreements and related reporting.
- Provides input into and administers assigned budget, ensuring that expenditures are controlled and maintained within approved budget limitations.
- Plans and leads complex projects and develops and manages project plans, including program and capital improvements.
- Develops and maintains project documentation including templates, spreadsheets, reports, briefing notes, presentations
- Leads or participates in working/advisory groups in project planning and program development processes by identifying, formulating and recommending new program features, and preparing and supporting the delivery of implementation plans.
- Represents Toronto Public Health or supports senior leaders as representatives in meetings with a wide range of internal and external stakeholders (i.e. City
- Divisions, community groups, other levels of government, agencies, senior staff, and elected officials).
- Develops communication materials, makes presentations and brokers knowledge transfer
Key Qualifications:

Post-secondary education in a relevant professional discipline (i.e. Project Management, Business, Information & Technology, Health, Education, Planning etc.) or an equivalent combination of education and/or relevant experience.

Considerable experience in project management and leading complex projects involving a wide range of internal and external partners.

Considerable experience facilitating staff teams, committees or work groups.

Considerable experience in gaining buy-in and support from Managers and Senior Leaders on new approaches, change initiatives and implementation of health-related best practices.

Experience leading capital projects or implementation of clinical programs is an asset

Highly developed human relations, consultation, facilitation and negotiation skills at all levels within the organization.

Ability to maintain accurate and detailed documentation and records.

Highly developed written and oral communication skills, including experience writing reports, briefing notes, and delivering presentations.

Conceptual thinker with ability to create new concepts that advance the program.

Ability to work in multidisciplinary teams and collaborate with other City Divisions and external partners.

Ability to take initiative and to work independently with competing demands and high-performance expectations to set and exceed goals.

Knowledge of relevant issues and trends as related to homelessness and addictions programs, including recovery and treatment

Knowledge of collective agreements, guidelines, policies, procedures and relevant legislation including documentation standards, Information Management, Employment Standards Act and Occupational Health & Safety Act.
