Reporting to the Tuberculosis (TB) Program Supervisor, the TB Program Registered Practical Nurse will be responsible for working in the community with clients who have TB infection or disease, to support them through their TB treatment. Also, investigating persons who are placed on medical surveillance, or treatment for Tuberculosis (TBI) infection.

  • Work independently and with a multidisciplinary team to provide direct care to clients in their home and/or other community settings such as schools, daycares, workplaces and shelters.
  • Provide clinical services under medical directives, such as administering TB medication for TB disease and those with TB infection via directly observed therapy in the community and remotely via video conferencing; plant and read TB skin tests; monitor for side effects; assess client's response to TB treatment and provide immediate care/action in emergency situations and follow up with clients and health care providers.
  • Arrange referrals, collect, transport and process clinical specimens and accompany clients to appointments.
  • Conducts telephone/in-person assessment for TB signs/symptoms, health history, and TB risk factors.
  • Educate, counsel and support clients and their families regarding Tuberculosis and other social and health issues.
  • Maintain accurate health records and documents as per program standards, both written and electronic.
  • Communicate regularly with Health Care Providers about the client's health issues.
  • Attends team meetings, participates in training and peer support activities.
  • Maintains statistical information.

Key Qualifications:

Your application must describe your qualifications as they relate to:

  1. Considerable experience as a licensed Registered Practical Nurse in the Province of Ontario in the field of community health, public health and/or Communicable Disease Control.
  2. Completion of the Registered Practical Nurse Administration of Medications course and considerable experience in the provision of oral medications, and subcutaneous injections.
  3. Experience conducting assessments and provide teaching and counselling to individuals and families specifically to those who have recently settled as new immigrants/refugees, homeless and under housed clients and able to handle complex situations and environments.
  4. Experience in TB prevention and control and accompanying issues.
  5. Considerable experience using computer-based health information systems and databases, e.g. Ontario Telemedicine Network (OTN), Connecting Ontario (CO), and Electronic Medical Records (EMR), and utilizing a variety of Microsoft Office software programs, e.g. Outlook, Word, Excel PowerPoint.

 

  1. Must possess a valid Ontario Driver's License Class "G" with access to a vehicle and able and willing to conduct daily community visits within the Greater Toronto Area.

You must also have:

  • Fluent in Cantonese, Mandarin, Vietnamese, Korean, Arabic, Urdu, Tamil, Punjabi, Somali, Tigrinya, or Amharic will be considered an asset.
  • Ability to perform various nursing procedures timely and accurately.
  • Ability to work independently and communicate effectively in a team environment.
  • Strong organizational skills and accurate record keeping ability.
  • Ability to respond and provide customer service over the phone, email, or in-person
  • Excellent interpersonal skills with the ability to establish and maintain good personal and public relations.
  • Ability to work flexible hours, which may include overtime, shift work, early mornings, evenings and weekends depending on client needs and the nature of the Public Health emergency.
